A scapholunate dissociation (SLD), which progresses through several stages, finally ends as a perilunate dislocation. The injury can be completely ligamentous or can be a combined fracture and ligamentous injury. SLD is one of the most common types of carpal instability. Often overlooked, it may lead to weakness and arthritis.

The scapholunate interval is the radiographic measurement of the scapholunate joint on PA wrist projections. Abnormal widening is indicative of injury to the scapholunate ligament that occurs with scapholunate dissociation . 2015-12-21 These findings represent scapholunate dissociation (rotatory subluxation of the scaphoid), and is the mildest form of this injury. Case B-D. Case 7B showed a transcaphoid perilunate dislocation; note that the lunate and radius remain normally aligned. This injury is 2-3x more common than a lunate dislocation.

SLD can cause osteoarthritis of the wrist. Recently a study was published that shows cineradiography to be an excellent radiological imaging technique for diagnosing SLD at a tertiary centre for hand and wrist Scapholunate Advanced Collapse (SLAC wrist) is due to advanced injury of the scapholunate and volar radioscapholunate ligaments which may lead to rotatory subluxation of the scaphoid and scapholunate dissociation. As in this severe case, the capitate may migrate proximally into the widened scapholunate space and articulate with the distal radius.
